About the role
The National Syrian Project for Prosthetic Limbs (NSPPL) is seeking a dedicated Case Manager to join its Reyhanlı Center in Hatay, Turkey. This 8‑month fixed‑term position involves providing comprehensive case management for patients requiring prosthetic limbs and rehabilitation services.
Key responsibilities
- Conduct initial assessments of patients’ medical, psychological, living and social conditions.
- Determine individual needs and develop tailored treatment plans in coordination with other departments.
- Provide individual sessions at the center and conduct field visits when necessary.
- Follow up on treatment plans to ensure continuity of care.
- Regularly update referral cases and maintain accurate case records.
Required profile
- Experience in case management or similar patient‑centered roles.
- Ability to assess medical and psychosocial needs.
- Strong coordination and communication skills with multidisciplinary teams.
- Willingness to work on‑site at the Reyhanlı Center for the contract duration.
