About the role

The pharmacist will drive value creation, quality improvement, and cost control within value‑based reimbursement programs. Working directly with primary care provider (PCP) offices, the incumbent may also interact with patients on site, applying deep expertise in clinical pharmacy and medication management.

Key responsibilities

• Engage PCPs and other providers to meet fiscal and ROI targets.
• Apply clinical pharmacy knowledge to formulary management, benefit design, and policy development.
• Develop and implement medication adherence strategies, Medication Therapy Management (MTM), and immunization programs.
• Collaborate in a matrix environment with data analytics, clinical teams, and internal/external stakeholders.
• Utilize utilization management and specialty pharmacy expertise to support provider decisions based on data analysis.

Required profile

• Degree in Pharmacy.
• 3‑5 years of experience in clinical practice care planning.
• Proven experience in continuous improvement, utilization management, and managed care.
• Strong background in healthcare management and provider engagement.
