Medical Record Reviewer (Remote)

About the role

The Medical Record Reviewer will evaluate inpatient adult medicine documentation and related data to support AI model training and improve data quality. This remote contract position offers flexible hours ranging from 10 to 40 per week.

Key responsibilities

• Review medical records, including FHIR data and clinical notes, for AI training.
• Verify that clinical information and claims are supported by the medical record.
• Assess inpatient documentation such as HPI, PMH, medication reconciliation, social history, assessments, and plans.
• Identify missing, incorrect, or inconsistent information, including dosing adjustments, medication interactions, and age‑appropriate details.
• Provide feedback to enhance AI models and overall data quality.

Required profile

• MD or DO, board certified in Internal Medicine (or Internal Medicine with a Hospital Medicine focus).
• Strong understanding of inpatient documentation and hospital workflows.
• Experience reviewing clinical notes and raw medical record data.
• Prior work in chart review, clinical documentation improvement, utilization review, or medical coding/QA is a plus.
